Job Title: Independent Living Assessor

  • Salary Range: £31,716 - £41,286 per annum
  • Location: Westminster City Hall, 64 Victoria Street, Westminster, SW1E 6QP
  • Hours: 36 hours per week
  • Vetting Requirements: Enhanced DBS Check

The Role: As an Independent Living Coordinator within our Complex Care Services Review Team, you will hold a pivotal role in assisting adults with complex health and social care needs to maintain independence within their homes. Your duties will encompass:

  • Assessing individual requirements
  • Orchestrating support services
  • Advocating for optimal outcomes for our service users

Moreover, you will be instrumental in:

  • Identifying and implementing assistive technologies to enhance autonomy and cultivate an enhanced quality of life.

About You:

  • Possess GCSE Maths and English or equivalent qualifications
  • Hold a Level 3 Diploma/NVQ in a relevant field
  • Demonstrate proficiency in care coordination principles and the Trusted Assessor role
  • Display comprehensive knowledge of disabilities and conditions impacting individuals' autonomy
  • Capable of collaborative work within a multi-disciplinary team
  • Exhibit strong communication abilities and adeptness in engaging with diverse communities
  • Bring experience in assisting physically disabled or elderly individuals, preferably within a Local Authority context
  • Show proficiency in utilising computer databases (e.g., Mosaic)
  • Possess financial acumen to facilitate individuals with Direct Payments
