We are seeking a dedicated Mental Social Worker to join our adult services team, providing support to adults suffering with ill mental health. This role is pivotal in conducting assessments, ensuring safety and well-being, and coordinating comprehensive care. Day to Day of the Role:

Conducting swift and thorough assessments to understand the individual’s situation and needs.

Determining and executing immediate actions to ensure the person’s safety and well-being.

Coordinating with other services to provide comprehensive care.

Delivering planned care as defined by team coordinators, including health and social care support.

Responding to escalations of need and supporting individuals to connect with their communities.

Reducing isolation and planning for future needs.

Required Skills & Qualifications:

2+ years experience working in a mental health team as a social worker or Care Coordinator.

Excellent communication skills, with a focus on person-centred care.

Commitment to delivering best practice in social work, promoting professional values and standards.

Must have completed their Assessed and Supported Year in Employment (ASYE) and be 2 years post-qualified.

Current registration with Social Work England is required for qualified social work posts.
