Job Description:

Are you an experienced social worker passionate about transforming adult social care? We are seeking a motivated and skilled individual to join the Adult Transformation East and West Team in Middlesbrough.
In this role, you will play a pivotal part in assessing needs, developing innovative care plans, and working collaboratively to implement transformative solutions for adults.

Responsibilities:

  • Conducting comprehensive assessments of adults' needs, strengths, and risks.
  • Developing and implementing personalized, innovative care plans that promote independence, well-being, and transformation.
  • Providing ongoing support and intervention to adults and their families, ensuring access to necessary services and resources.
  • Collaborating with healthcare professionals, community organizations, and other relevant agencies to coordinate comprehensive and transformative care.
  • Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.

Qualifications:

  • Qualified Social Worker registered with Social Work England.
  • Proven experience working with adults in a social care setting.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with service users and their families.
  • Knowledge of relevant legislation, policies, and procedures related to adult social care.
