About the Role:
- Provide high-quality social work support to adults within a hospital environment, facilitating timely and appropriate discharge planning.
- Conduct thorough assessments, identifying individual needs and developing person-centered care plans to support safe and effective transitions from hospital to community settings.
- Collaborate with multidisciplinary teams, including medical staff, nurses, and other healthcare professionals, to coordinate care and advocate for patient well-being.
- Liaise with families and carers, providing support and guidance throughout the hospital stay and discharge process.
- Contribute to a supportive and collaborative team environment focused on delivering exceptional patient care.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a Social Worker.
- Experience working within a hospital or acute care setting is preferred.
- Excellent communication, assessment, and case management skills.
- Strong understanding of health and social care legislation and pathways.
- Commitment to person-centered care and ethical practice.
