We have an opportunity for a temp to join a Mental Health Hospital Discharge Team in Sheffield City Council.
Based at Michael Carlisle Centre and across SHSC Trust work sites, with 1-2 days a week from home if wanted.
The candidate will need to be an experienced Social Worker with a mental health background.
Core tasks and duties as follows:
- Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
- Some mental capacity assessments, BI decisions and safeguarding as required
- Attending 117 meetings, ward rounds, professional meetings
- Facilitating learning opportunities for ASYEs through shadowing
- Participation in duty rota
The successful candidate will be welcomed and integrated into the team and offered regular supervision and support from a 'hands-on’ manager

- DIPSW or equivalent qualification in Social Work
- Social Work England registration
- Enhanced and in date DBS certificate.
- Excellent communication skills and be able to make good decisions based on information available.
- Can work alone and within/contribute to a team/multi-agency set ups
- Possess knowledge of service needs.
- Competent IT skills and be able to navigate electronic file/recording systems.
- Can write clear, concise, and robust assessment/reports/files.
- Have a flexible approach to work and working hours when the need arises
