Job Title: Adults Social Worker - Transformation and Learning Team

Location: Middlesbrough

Salary: £37 per hour

We are seeking an experienced and dedicated Adults Social Worker to join our Transformation and Learning Team. This role involves working with adults with learning disabilities, including those in transition to adult services and those over 18. The position requires an individual with a strong commitment to improving outcomes for adults with learning disabilities through effective case work and reviews.

Responsibilities:

  • Manage a diverse caseload of adults with learning disabilities, ensuring high standards of practice and compliance with statutory requirements.
  • Conduct comprehensive assessments, develop and implement care plans, and monitor progress to ensure the well-being and independence of service users.
  • Perform face-to-face visits five days a week to assess needs, provide support, and review care plans.
  • Work from an office setting two days a week to complete necessary administrative tasks, maintain accurate records, and collaborate with colleagues.
  • Engage with service users, families, carers, and other professionals to provide holistic support and interventions.
  • Undertake case reviews, ensuring that care plans are up-to-date and reflect the current needs and goals of service users.
  • Advocate for the rights and needs of adults with learning disabilities, ensuring their voices are heard and respected.
  • Maintain detailed and accurate records of assessments, plans, and reviews.

Requirements:

  • Registered with Social work England
  • A Degree in Social work or Diploma in SW, CQSW, CSS
  • Right to Work in UK
  • Enhanced DBS check, dated within the last 12 months
  • A full UK driving licence
  • Minimum of two years post-qualifying experience in adult social work.
  • Experience working with adults with learning disabilities, particularly in transition to adult services.
  • Knowledge of relevant legislation, policies, and procedures.
