Our Client, Swindon Borough Council, is seeking two Adults' Social Workers to join their HomeFirst and Urgent Response Team as well as the Discharge hub Team.

Responsibilities of this role:

  • We have 2 positions available.

  • One position is within the Homefirst and Urgent Response Team.

  • The second is in the Discharge Hub team who support onward discharge planning from the acute hospital or a discharge to assess bed.

  • Both are busy and fast paced teams and would require a dynamic worker with experience supporting hospital discharge and admission avoidance.

  • The team are based in the office 2 days a week but expected to complete urgent visits at short notice.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have minimum of 2 years’ experience.
