Job Overview:
We are seeking an experienced Band 6 Occupational Therapist to join the Early Supported Discharge (ESD) team at Ashford Hospital. The role involves supporting patients in transitioning safely and effectively from acute care to home, with a focus on stroke and neurological rehabilitation.
Key Responsibilities:
Carry out comprehensive assessments for patients ready for early discharge
Design and deliver personalised rehabilitation plans in the community setting
Work closely with the MDT to coordinate smooth discharges and follow-up care
Provide patient education, carer training, and ongoing therapy reviews
Maintain accurate documentation and contribute to service improvement
Requirements:
HCPC registered Occupational Therapist
Previous NHS experience in stroke or community rehabilitation (minimum 6 months)
Strong clinical reasoning and assessment skills
Ability to work independently in patients' homes and across community settings
Full UK driving licence and access to a vehicle (desirable)
