Care Assessor Adults

Contract - 3 - 6 month agency role with a view to being extended on an ongoing basis.

Working Hours - Full Time Position (36 hours).

Locations - RM7.

Rate of Pay - £19 - £23 per hour - Higher rates can be negotiated for more experienced candidates.

Main Duties:

• Undertake assessments, support planning, identification of resources and reviews in order to assist in maintaining the independence of adult social care service users.
• Complete appropriate assessment forms, create and maintain records, reports and letters.
• Identify if there is a need requiring referral to Adult Social Care or Health staff for a specialist assessment and refer as necessary.
• Co-ordinate and review ongoing packages of support and to provide care co-ordination role for service users as appropriate.

Role Requirements:

• Case Load/Duty expectations - (Varied) - depending on level of experience, 20-25 cases.

Complexity of cases:

- For Care Assessors- knowledge of Safeguarding.
- Working with clients with various social care needs and some complexities.

- Transition work-younger adults.
