Job Description


What You’ll Do:

As a Field Care Supervisor, you will play a crucial role in ensuring we deliver the highest quality care support services. Your responsibilities will include:

  • Supporting Care Workers: Provide practical supervision, instruction, and support to our dedicated care workers to ensure they meet the highest standards of care.
  • Assessment and Review: Conduct assessments and reviews of individual services, including regular needs and risk assessments, to ensure our service users receive the best possible care.
  • Care Planning: Collaborate with Care Coordinators to plan effective and realistic care worker rotas that align with the specific needs of our service users and the skills of our care workers.
  • Performance Monitoring: Perform on-site observations and evaluations of care worker practices as part of routine supervision or in response to performance concerns.
  • Multi-Disciplinary Collaboration: Participate in multi-disciplinary reviews of service users’ needs to provide comprehensive and coordinated care.
  • Training and Mentorship: Shadow new care workers in the field to ensure their competence and confidence in delivering exceptional care.

While the majority of your time will be spent in the community, you’ll also handle some office-based administrative tasks to support our care operations.

Qualifications


What You Need:

  • Assessment Experience: Proven experience in conducting initial risk and needs assessments for individual service users, whether in their homes or in other settings such as hospitals, residential, or nursing homes.
  • Collaborative Approach: Ability to work in partnership with service users, their families, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of comprehensive, holistic care packages.
