What You'll Do:

• Conduct assessments and create personalized care plans for patients
• Visit patients in their homes, communities, or hospitals
• Collaborate with our care team to ensure the best outcomes for patients
• Provide ongoing support and supervise care packages
• Participate in reviews and multi-sign-offs (with extra compensation)

What We Need:

• A qualified Registered Nurse
• Strong assessment and care planning skills
• Excellent communication and teamwork abilities
• Flexibility to travel between locations
• Ability to work independently
