Role Overview:

We are partnering with a progressive Primary School in Swindon dedicated to providing exceptional support to children with diverse and complex needs. As the EHCP & Referral Coordinator, you will play a crucial role in managing Education, Health, and Care Plans (EHCPs) and coordinating referrals to ensure students receive the tailored support they need.

What You’ll Be Doing

  • Collaborating with parents, carers, and professionals to gather information about a child's SEND
  • Conducting assessments and reviews to determine the child’s needs
  • Facilitating multi-disciplinary meetings to develop EHCPs
  • Acting as the central contact for all EHCP-related communications
  • Coordinating assessments, reviews, and meetings for timely information exchange
  • Maintaining accurate records and documentation
  • Liaising with educational professionals to implement EHCP provisions
  • Monitoring and reviewing the child’s progress, ensuring EHCP provisions are met
  • Participating in annual reviews and evaluating EHCP effectiveness
  • Providing reports and updates to Local Authorities and relevant bodies

About You

  • Be able to do the job as described
  • Familiarity with the EHCP assessment and planning process
  • Proficient in assessing the needs of children with SEND
  • Excellent communication and interpersonal skills
  • Strong organisational and time management skills
  • Ability to collaborate effectively with various professionals and agencies
