Central London Community Healthcare NHS Trust Health Visitor.

Queen's Park Family Hub, 88 Bravington Road, W9 3AL

Band 6 Health Visitor

Pay rate:

NHS capped rates - Band 6.

Health Visitor Responsibilities:

Leading and coordinating the healthy child programme by carrying out the 5 mandated contact visits. Providing a seamless service to universal and vulnerable children and families by working in partnership with multidisciplinary teams. Early identification, intervention, and appropriate onward referral. Good communication skills both written and verbal.

Contract of Health Visitor:

Monday - Friday 9-5pm
