Key Responsibilities:

• Conduct Initial Mental Health Assessments and develop Support Plans
• Understand and manage Delayed Transfer of Care (DTOC) cases
• General Case Management and needs assessments

What We're Looking For:

• Qualified social worker with 2+ years of experience in adult mental health (required)
• SWE registration (required)
• Professional qualification in Social Work (required)
• Understanding of hospital discharge processes and DTOC (required)
