Leaving Care Personal Advisor

Shropshire Council are looking for a Leaving Care Personal Advisor to cover maternity leave. They have an office base in Shrewsbury but this is a hybrid post and the successful applicant will be provided with all the equipment needed to support working from home.

About the Role

The Leaving Care Personal Advisor will hold a case load, ensuring the UASC/Asylum seeking Care Leaver is at the centre of all assessment, planning and support provided. Duties will involve completion of needs assessments, development of the pathway plan, which will include triple planning, review of the plan as appropriate and where significant events occur. Liaison with social workers, other professional agency colleagues, such as housing, probation, or the Police, and solicitors who are assisting and providing services for the Care Leaver.

The Personal Advisor where requested will provide advocacy for the Care Leaver to ensure their health, education or training/employment, accommodation and financial help, family relationships, religious, culture, legal issues and race needs are being met.

Key Responsibilities

  • To hold a caseload of young adults who are Asylum seeking Care Leavers.
  • To complete Needs assessments.
  • To complete with the care leaver their Pathway Plan.
  • To advocate for Care Leavers with other statutory agencies.
  • To attend Court appointments or hearings where requested by the Care Leaver.
  • Attend meetings with other agencies that review and respond to the presenting issues of the allocated Care Leaver.
  • To assist with the setting up of Care Leavers in independent accommodation, with financial and practical arrangements.
  • To visit the Care Leaver where they are living.
  • To assist, advice and befriend the Care Leaver to ensure their transition to adulthood develops independence, knowledge and skills.
  • Work with care leavers who may be homeless and require accommodation alongside housing colleagues within the Council and from private providers.
  • To work with social workers in Children's, Adults and Mental Health services to achieve best outcomes for the Care Leaver.

Requirements

  • A good understanding of the needs of young people with care experience
  • Knowledge of benefits, housing, supporting asylum seekers, and education
  • Full UK driving license and access to a vehicle is essential
  • Enhanced DBS check
  • Minimum NVQ Level 3 in Children and Young People or equivalent qualification
  • Office hours are are 7am to 7pm Monday - Friday
