Job Description:
As a Senior Adults Mental Health Social Worker, you will play a crucial role in assessing, planning, and delivering support to individuals experiencing mental health challenges. You will work within a multidisciplinary team to provide holistic care, promote recovery, and enhance the quality of life for service users.
Key Responsibilities:
Assessment and Care Planning:
  • Conduct thorough assessments of individuals' mental health needs, including risk assessments and evaluations of social, emotional, and environmental factors.
  • Develop and implement individualized care plans in collaboration with service users, their families, and other professionals, focusing on recovery-oriented goals.
Direct Support and Intervention:
  • Provide therapeutic support, counselling, and crisis intervention to individuals experiencing mental health issues.
  • Advocate for service users' rights and needs, ensuring they have access to appropriate services and resources.
Multi-Agency Collaboration:
  • Work closely with healthcare providers, community services, housing agencies, and other relevant organisations to coordinate and deliver integrated care.
  • Participate in multidisciplinary team meetings, case conferences, and care reviews to ensure comprehensive and cohesive support.
Safeguarding and Risk Management:
  • Identify and respond to safeguarding concerns, following established protocols to protect vulnerable adults from harm.
Qualifications:
  • A degree in Social Work.
  • Registration with Social Work England.
  • Extensive experience in mental health social work, preferably with adults.
  • Strong assessment, care planning, and therapeutic intervention skills.
  • Excellent communication, collaboration, and advocacy abilities.
  • Knowledge of safeguarding protocols and risk management strategies.
