Are you an Occupational Therapist seeking a new challenge? Do you have previous experience supporting adults in a healthcare setting?
Service Care Solutions are working with North Lincolnshire Council to recruit a qualified Occupational Therapist to join the Integrated Discharge Team at Scunthorpe Hospital.

The successful candidate will focus on facilitating safe and timely patient discharges, working closely with multidisciplinary teams to ensure seamless transitions from hospital to home or other care settings.
Contract Duration: 5 August 2024 to 4 October 2024, with potential for extension
Hours: Full-time, 37 hours per week
Shifts: Monday to Friday, 09:00 - 17:00
Location: Scunthorpe
Pay: £30.00 p/h LTD

Key Responsibilities:

  1. Discharge Planning and Coordination:
    • Collaborate with the Integrated Discharge Team to facilitate safe and timely discharges from Scunthorpe Hospital.
    • Conduct comprehensive assessments of patients' functional abilities, needs, and home environments to develop individualised discharge plans.
    • Work closely with multidisciplinary teams, including doctors, nurses, social workers, and community services, to ensure seamless transitions from hospital to home or other care settings.
  2. Patient and Family Support:
    • Provide education and support to patients and their families regarding adaptive techniques, equipment, and community resources to enhance post-discharge independence.
    • Develop and implement therapeutic interventions aimed at improving patients' functional outcomes and quality of life.
    • Ensure that patients and their families are fully informed and involved in the discharge planning process, addressing any concerns and providing ongoing support as needed.

Essential Criteria:

  • Qualifications:
    • Degree in Occupational Therapy or equivalent.
    • HCPC registration.
  • Experience:
    • Proven experience in discharge planning and care transitions from hospital to home.
    • Experience working with adults in a healthcare or social care setting.
