OUTREACH SUPPORT WORKER

An Outreach Support Worker role is available to work with mental health needs in Tower Hamlets, London .

Within this role candidates would be required to provide home support visits, accompaniment on trips and day-to-day activities and also to appointments to help manage the service user’s daily life.

Ideally candidates would have experience working as an Outreach support worker, as well as mental health issues and learning disabilities.

Candidate should have an in depth knowledge of:

• Housing Benefits
• Pensions
• Job centre application
• Signposting to other agencies
• Would have worked with elderly and should be able to travel the clients to their homes
• Have solid IT skills (Outlook, Excel)
• Very organised and able to manage a high caseload Desirable.
• Should be able to drive a car

Working hours: Mon-Fri9am-5pm

You will be working with a supportive team covering the Borough of Tower Hamlets. Its an office based role when not in the community.
