4Recruitment Services are seeking  a Patient Discharge Officer for our client located in Northampton.

The post holder will be working in an integrated multi professional patient flow team and will be involved facilitating a safe and timely discharge for all patients on the ward.   The post holder will co-ordinate timely patient referrals to relevant community services and liaise directly both with patients and their relatives as required about discharge arrangements.

The post holder will be expected to facilitate discharges over the 7-day period to meets the needs of the patients.  
  
DUTIES & RESPONSIBILITIES INCLUDE:
  • Attending ward MDT meetings
  • Attending board rounds and co-ordinating all actions relating to the process
  • Completing discharge notifications for onward referrals to other discharge services
  • Completing all referrals to other services such as community nursing etc and checking that medication for discharge is available
  • Ensuring all transport arrangements are in place, arranging home assessment visits, progress meetings.
  • The post holder will be expected to communicate with patients who may have communication difficulties, families and other colleagues.
  • Post holder will ensure all patients documentation and pathway is documented electronically and that protocols are followed for data collection purposes.
  • The post holder is required to input all discharge and admission data to support KPI and monitoring.
  • The post holder will be involved in the induction of junior colleagues and provide support and guidance as necessary.
  • Support complex discharge home assessments if required
  • Proactive management for patients with no reason to reside
  • Attend the service tracking meetings, providing patients updates and progress and co-ordinating any actions arising
  
ESSENTIAL REQUIREMENTS:
  • Foundation degree in health-related subject and/or relevant experience working at this level is essential.
  • Experience working within a Healthcare setting preferably the NHS is essential.
  • Functional skills in literacy and numeracy to level 2 or equivalent is essential.
  • Experience working in a rehabilitation setting preferably at an inpatient unit is desirable.
  • Approachable nature to patients and colleague and able to deal with interruptions/queries throughout the day
  • Provide discharge support to patients in a timely manner
  • Excellent IT skills with ability to use different electronic systems
  • Ability to work without direct supervision
