We are seeking a Band 6 (equivalent) Physiotherapist to work within the Reablement Team in adult social services.

Based across North Somerset. There are offices to work out across the county, as well as working from home options.

Full-time hours (Mon-Fri)

The TRI team provide a short term therapy led, TEC first Reablement service for people within hospital discharge pathways and in the community.

  • Role entails a varied caseload providing case management and specialist Physiotherapy input alongside domiciliary care providers and care home staff (residential reablement).
  • Day to day tasks include goal setting, discharge planning, equipment provision, specialist assessments and interventions including falls risk assessments.
  • TRI team promote values around innovation, risk enablement, empowering individuals and maximising independence. Candidates need to have some awareness of Technology Enabled Care devices and/or be willing to learn to utilise these within own practice.

Requirements:

  • Minimum 2 years post-graduate Physiotherapy experience
  • Experience in community rehab or social services
  • Car driver
