Responsibilities of this job role
- To perform advanced holistic triage and assessment of patients with multi-system symptom presentation associated with complex physical, emotional and psychological health
- To develop and deliver an individualised treatment programme
- Where appropriate to work collaboratively with primary and secondary health care services, third sector services and social prescribing, education and social services, to ensure the delivery of a seamless and high quality service.
- To be responsible for a caseload and work without direct supervision. Indirect supervision from the clinical lead will be via regular formal training, 1:1 sessions, peer review and case studies. Access to advice and support from a clinical or team lead and MDT is available
- To provide support to MDT and cover for absent colleagues as required.
- To undertake all aspects of clinical duties as an autonomous practitioner.
- To act as an advocate for patients and their families when necessary.
- To carryout risk assessment in line with Trust/National Protocol.
- To anticipate care needs and develop individual patient centred intervention and management plans
- To ensure patient care and planned therapeutic interventions are supported by available evidence base and experiential knowledge, and are in accordance with Code of Professional conduct.
- To keep up to date with professional standards, keep abreast of recent literature and adhere to NICE guidance
To be successful in applying for this role you will need:
- Previous experience as a Band 5 or 6 Physiotherapist
- Experience working with conditions with complex symptom presentation
- Experience working with individuals with Long Term Health Conditions and conditions within allocated specialities
- Experience supporting junior staff, support staff
- HCPC registered.
- Degree In Physiotherapy or equivalent.
