Practice Lead

Service: Covering our East Anglia Service

Hours: 30 Hours PW

Salary: £25,500 PA Pro Rata

Who are Ambient Support?

Ambient Support, a registered UK charity with over 25 years' experience in providing care and support services for older people, people with a mental health needs and people with learning disabilities.


About our Practice Lead Role:

Working closely and as part of the North Division you will ensure that the people we support have the highest standards of care and have comprehensive support plans along with relevant risk assessments. The support plan for each person must be appropriate, meet their needs, and reflect their preferences. You will support each team to work in line with the support plan and risk assessments so that quality standards are maintained. To be a champion for exceptional care and support; including a person-centred ethos in every aspect of the role.

Key Responsibilities of our Practice Lead Role:

* Carrying out, collaboratively with the person and their circle of support, an assessment of their needs and preferences before moving into their home

* Developing, collaboratively with each person, a support plan care with a view to achieving persons preferences and ensuring their needs are met

* Enabling and supporting each person to understand the care and health treatment choices available to them, with a competent health care professional or other competent person, the balance of risks and benefits involved in any particular course of treatment

* Enabling and supporting relevant persons to make, or participate in making, decisions relating to the persons care or treatment

* Providing opportunities for relevant persons to manage each person's care or treatment

* Involving relevant people in decisions relating to the way in which the regulated activity (personal care) is carried on in so far as it relates to each person's care or treatment

* Providing relevant persons with the information they would reasonably need for the purposes

* Making reasonable adjustments to enable the person to receive their care or treatment

* Where meeting each person's nutritional and hydration needs, having regard to their well-being

* Leading on mental capacity assessments where appropriate

* Supporting people to make best interest decisions with their circles of support

* Where a person we support has a deprivation of liberty (DoLs) in place ensure that clear information is in the persons care plan and that those supporting the person understand what restrictions are in place

* To keep a up to date log of details relating to each DoLs across the division

* To complete restrictive placements assessments on each person we support and actively work to reduce known restrictions where possible for each person

* Taking an active part in the shared on-call for the Division.


Qualifications/Education/Training:

* Desirable NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards

* Proficient in MS Office and web-based services

Experience:

* Experience of managing a care or support related service

* A commitment to delivering person centred approaches personalised support

* Can identify innovative practice and share learning effectively with colleagues

Skills and Knowledge:

* Flexible, adaptable, and willing to face up to changing circumstances and new opportunities

* Highly self-motivated, proactive and motivated towards excellence and improvement

* Ability to work on own initiative and to established deadlines

* Ability to evaluate information, assess risks, identify concerns and make recommendations

* Excellent written, oral, listening and communication skills

* Willing to travel to different areas as required

* Working flexibly in accordance with needs of post, occasional evenings and weekends

* Able to work as part of a dispersed team of colleagues
