Senior Nurse Assessor

Service Care Solutions are looking for Senior Nurse Assessor to work a Temporary contract in NHS in Birmingham.

Working
Full time (Mon to Fri 9-5) with one day at home.

CANDIDATES MUST BE ABLE TO DEMONSTRATE RECENT EXPERIENCE OF WORKING IN CHC.

• The post holder will work jointly with health, social care and independent sector colleagues to ensure care management processes are appropriate and at time may be asked to present assessments to the continuing care panels for ratification.
• To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, co-ordination and participation in MDT eligibility recommendations to be verified by ICB decision makers.
• The post holder will be responsible for complex clinical caseload comprising of; Assessments, undertaking of review (3/12 CHC review unscheduled review and Fast Tracks and implementation on Personal Health Budgets and ongoing Case Management of Patients funded by Continuing Healthcare (CHC), Mental Health & Learning Disabilities.
• Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care.
• To support the seamless implementation of the National Framework for Continuing Healthcare including contribution to the development of training programmes and delivery plans as required to facilitate others learning.
• To work with the Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care Applications. Influence best practice by providing professional nursing advice on a range of physical and mental health nursing issues, which will require the individual to work in collaboration with partners and other professionals to ensure patients' health care needs are met appropriately and effectively.
