Social Worker - Children with Disabilities Team

Location - Stockton-on-Tees Borough Council, Stockton-on-Tees

Stockton-on-Tees Borough Council is dedicated to delivering exceptional services to our community. Their Children with Disabilities Team is committed to providing compassionate and comprehensive support to children with disabilities and their families. They are seeking a motivated and dedicated Social Worker to join their team and help us make a positive difference in the lives of children and young people.

As a Social Worker in the Children with Disabilities Team, you will play a vital role in assessing the needs of children with disabilities and creating tailored care plans to support their development and well-being. You will work closely with families, carers, and other professionals to ensure a holistic approach to each child's care.

Responsibilities -

  • Conduct assessments of children with disabilities to determine their needs and develop appropriate care plans.
  • Provide direct support and interventions to children and their families, ensuring a child-centered approach.
  • Collaborate with multi-agency teams, including healthcare providers, educators, and community organisations, to coordinate comprehensive care.
  • Advocate for the rights and needs of children with disabilities, ensuring they have access to necessary resources and services.
  • Maintain accurate and up-to-date case records, ensuring compliance with statutory requirements and best practice guidelines.
  • Participate in regular team meetings, supervision, and continuous professional development opportunities.

Qualifications and Experience

  • A degree in Social Work and registration with Social Work England.
  • Experience working with children with disabilities and their families is highly desirable.
  • Strong understanding of relevant legislation, policies, and best practices in social work and disability services.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with children, families, and professionals.
  • Ability to work independently and as part of a dynamic team, managing a diverse and demanding caseload.
