Job Title: Supported Pathway Officer

Pay Rate: £25 P/H
Job Type: Full Time/Contract

Shift pattern is Mon-Fri 8am-4pm or 12-8pm- You will either be doing an early shift for the week which is 8-4pm or lates 12-8pm

The Role

Assist residents in securing their next stage housing, providing resettlement support so that they successfully move into and sustain their new home. To provide appropriate floating support for 6 months and to ensure they are linked into necessary support services thereafter.

To provide assertive client centred key-working, risk assessment and support planning to a caseload of approximately 10 clients aimed at supporting them to maintain their current accommodation and link into services and meaningful activities which help them to improve their physical and mental wellbeing well-being and life skills

The Candidate

• Knowledge of the impact of Adverse Child Experiences and the principles of Trauma Informed Care when dealing with homeless people who have complex needs
• Knowledge of the care act and housing legilslation and the avalible support in relating to single homeless adults.
• Knowledge of welfare benefits and other support that is avalibale in relation to single homelss adults.
