The Bromley by Bow Centre (BBBC) is launching an exciting new social prescribing service for young people in the Northeast locality of Tower Hamlets. This is a specialist link worker role using a youth work approach to offer tailored support to young people.

Key to this role is one-to-one person-centred support and building trusted relationships. It will act as a bridge between health services and the community to support and empower young people to access opportunities and take positive steps to improve health and wellbeing outcomes.

The Youth Social Prescribing Link Worker will connect young people to a broad range of services and interventions in their local area in a timely and effective way. The role will be part of the Centre’s social prescribing team, utilising their knowledge, experience and support. A strong knowledge of local organisations and services for young people will be essential.

The successful candidate will have a background in youth work and experience in engaging young people from diverse backgrounds. The role offers an exciting opportunity to co-design and codevelop a new social prescribing service - shaped by young people, for young people.
The social prescribing service will support young people through:

One-to-one-person centred support focussing on what matters to the individual Addressing the social determinants of health, such as mental health, wellbeing, education, employability and social welfare advice.

Opportunities to build social connections within their local communities, fostering engagement in fun and meaningful activities.

Emotional support to build resilience and confidence.

Guidance on navigating and accessing wider support services.

You may also have experience in the following: Youth worker, Social Welfare, Social Liaison, Social Worker, Social Prescribing Link Worker, Advise, Counselling, Not For Profit, Charity, Advocacy, Advice Worker, Service Coordinator, Project Coordinator, Service Delivery, etc.
